Job summary

Transitional Care Management in Illinois is seeking an MDS Coordinator (RN or LPN) to oversee interdisciplinary assessments and care plans. You will work with the Regional MDS Director to ensure high-quality resident care and accurate MDS/RAI documentation, while engaging residents and families.

Qualifications

• RN or LPN license required in Illinois.
• Current MDS 3.0 certification ideal.
• 1-2 years MDS experience in Skilled Nursing Facility preferred.

Responsibilities

• Coordinate and oversee interdisciplinary assessment and care planning.
• Maintain regular communication among care team, resident, and family.
• Ensure data collection for quality improvement and adherence to MDS/RAI requirements.
