Job summary

Rolling Hills Healthcare and Rehabilitation Center in New Jersey is seeking a Full-Time MDS Coordinator to join our clinical team. The ideal candidate will have strong communication and team skills and be proficient with PCC software to ensure timely MDS completion.

The role requires current NJ RN license and 1–2 years of MDS experience. You will collaborate with therapy and care teams to develop individualized care plans and maintain accurate documentation.

Qualifications

• Must have NJ RN license in good standing.
• Familiarity with Point Click Care (PCC) preferred.
• 1-2 years of recent MDS experience preferred.
• Strong communication and team collaboration skills.

Responsibilities

• Schedule and maintain accurate timely completion of MDS using PCC software.
• Ensure all MDS are in compliance with State and Federal regulatory requirements.
• Ensure timely and accurate electronic transmission to the state.
• Attend and manage weekly utilization review meetings with therapy and team members.
• Participate in developing and updating individualized Care Plans.
• Monitor facility staff regarding MDS processes.
• Input and update ICD-10 codes for residents.

Rolling Hills Healthcare and Rehabilitation Center is currently seeking an outstanding Full-Time MDS Coordinator to join our team of professionals. We're looking for a motivated, team-oriented MDS Coordinator. The right candidate must have good communication skills in order to work effectively with the Interdisciplinary Team and keep accurate, detailed records.

Familiarity with Point Click Care (PCC) and 1-2 years of recent MDS experience preferred.

Current NJ RN license in good standing required.

• Responsible for scheduling and maintaining accurate timely completion of MDS using PCC Software system.
• Ensure all MDS are in compliance with State and Federal regulatory requirements
• Ensure timely and accurate electronic transmission to the state
• Attend and manage weekly utilization review meetings and maintain open communication with therapy and team members
• Participate in developing/updating/reviewing/completion of individualized Care Plans
• Monitor facility staff in regard to MDS process, as needed
• Monitor facility QM reports to ensure proper corrective action can be implemented when potential problems arise
• Participates in facility meetings as requested and completes all other duties as assigned
• Responsible for inputting and updating appropriate ICD-10 codes for all residents

Range of pay 90,000-105,000 based on experience.
