MediLodge Kalamazoo is seeking an MDS Registered Nurse or Licensed Practical Nurse to support the MDS Coordinator with timely scheduling, completion, and transmission of MDS assessments. This role emphasizes patient-centered care, collaboration with the team, and accurate documentation.

The ideal candidate will have an RN license (CPR certified), at least one year of nursing experience, and a commitment to high-quality resident care in a skilled nursing setting.

Qualifications

  • RN license in the state of employment (CPR certification preferred).
  • One year of nursing experience preferred.

Responsibilities

  • Support MDS Coordinator with timely PPS/OBRA assessments and RAPS.
  • Assist with data gathering for assessments and care plans.
  • Monitor MARs, TARs, flow sheets, and physician orders for accuracy.
  • Contribute to care planning process and care conferences.
  • Perform nursing functions as required and support team members.

Summary

The MDS Registered Nurse or Licensed Practical Nurse is responsible for Assisting the MDS Coordinator with timely scheduling, completion, and transmission of MDS.

Qualifications
Education
  • Graduate of an accredited School of Nursing, RN preferred.
Licenses/Certification
  • Valid nursing license in the state employed.
  • Valid CPR certification.
Experience
  • One year of nursing experience preferred.
Job Functions
  • Monitors and guides the completion of PPS and OBRA assessments and RAPS.
  • Primary UR Coordinator.
  • Oversees gathering of information for assessments and care plans.
  • Monitors and audits MAR’s, TAR’s, flow sheets, and physician orders.
  • Assesses prospective residents in conjunction with other team members.
  • Obtains data warehoused reports and QI/QM reports.
  • Schedules and facilitates in patient/family care conferences.
  • Participates on various committees.
  • Inputs or supervises the input of MDS assessments, the transmissions to the state, and obtaining receipt of validation.
  • Manages the Care Planning process.
  • Performs the functions of a nurse when required.
  • Performs other tasks as assigned.
