The MDS/RAI Coordinator is responsible for ensuring the timely completion of accurate assessments and interdisciplinary care plans that meet Federal and State guidelines.

Responsibilities
  • Complete assessments, Minimum Data Set (MDS), and care plans for all assigned guests.
  • Monitor the completion of MDSs by other disciplines within prescribed timeframes.
  • Advise supervisors of incomplete or untimely assessments by other disciplines.
  • Ensure accurate, timely completion of MDS/RAPs/Triggers sheets for assigned guests.
  • Initiate care plans and activities to achieve the best outcomes for guests.
  • Inform the Director of Nursing of persistent issues related to non-compliant documentation.
Qualifications
  • Must hold a current, valid RN license or temporary RN license through the State Board of Nursing, or a compact RN license, prior to the date of hire.
  • CPR certification required.

Pay Range

USD $44.00 - USD $48.00 per hour
