Job summary

Thompson Health’s Continuing Care Center in Canandaigua, NY is seeking a motivated MDS/Quality Coordinator to oversee MDS coding, pre-admission screening, and the interdisciplinary care plan.

RN preferred; LPN with 5+ years MDS experience considered. Coordinates Medicare certification requirements and collaborates with the Director of Clinical Compliance and DON to support Thompson Health’s mission.

Qualifications

• Current nursing license required.
• RN preferred.
• LPN with 5+ years MDS experience would be considered.

Responsibilities

• Oversee MDS coding and development of the Interdisciplinary Plan of Care.
• Coordinate Medicare determination and certification requirements.
• Pre-admission screening and determination of skilled services.
• Set ARD in collaboration with the interdisciplinary team and notify Clinical Reimbursement Specialist.
• Educate and train staff in MDS processes and CMS/RAI guidelines.
