Job summary

Dyersburg Health and Rehabilitation Center is seeking a compassionate MDS Coordinator (RN) to join our team in Dyersburg, Tennessee. This part-time role (20 hours per week) focuses on accurate MDS assessments, care plan development, and compliance with CMS and PDPM guidelines.

The ideal candidate is an experienced registered nurse with strong analytical and communication skills, able to coordinate with interdisciplinary staff to deliver quality resident care and timely documentation.

Qualifications

• Registered Nurse (RN) license in Tennessee.
• Previous MDS experience in long-term care preferred.
• Knowledge of skilled nursing quality measures, restorative therapy and clinical review.
• Strong communication, organization and analytical skills.

Responsibilities

• Coordinate and complete MDS assessments in accordance with federal and state regulations.
• Ensure timely and accurate completion of MDS and care plans.
• Collaborate with team members to develop and update resident care plans.
• Monitor documentation to ensure compliance and support reimbursement opportunities.
• Participate in Medicare meetings and manage PDPM process.
• Create and manage medical codes for billing and recordkeeping.
• Monitor and analyze quality measures, participate in clinical reviews and support restorative nursing programs.
• Maintain knowledge of current CMS regulations and reimbursement guidelines.

Dyersburg Health and Rehabilitation Center is seeking a compassionate, professional MDS Coordinator to join our team in Dyersburg, Tennessee. As our MDS Coordinator, you will play a key role in assessing resident health, collecting and analyzing data, and developing individualized care plans that align with each resident’s needs and expectations. This position is ideal for an experienced nurse who enjoys a combination of clinical and administrative responsibilities, with a focus on quality care and compliance.

MDS Requirements

• Registered Nurse (RN) license in the state of Tennessee
• Previous MDS experience in long-term care is preferred
• Knowledge of skilled nursing quality measures, restorative therapy and clinical review
