Otterbein SeniorLife is seeking an MDS Nurse Transition Leader to guide MDS processes across locations. The role includes completing MDS duties andCare Plans, mentoring staff, and leading utilization reviews. The candidate will participate in special projects for the VP Clinical Quality and support transition leadership needs.

Strong MDS skills, BSN, and 5+ years in long-term care are required. This position offers travel up to 100% and comprehensive benefits through Otterbein SeniorLife.

Qualifications

• BSN degree required.
• Minimum 5 years of MDS experience in long-term care.
• Experience in post-acute environments is a plus.

Responsibilities

• Effectively complete the MDS and Care Plans.
• Discuss elder observations/concerns with the appropriate coordinator.
• Generate/update Monthly MDS Calendar and Notification Process for Support Staff.
• Provide coaching and direction to staff as needed.
• Complete certifications and recertifications for residents.
• Facilitate weekly Medicare/therapy Utilization Review Meetings.
• Act as the therapy liaison and provide training materials.
• Collaborate on new processes, forms, and systems.
• Be prepared for morning meetings and assist with survey processes when required.

Job description

This position is designed to serve as an MDS Nurse Transition Leader when an opening occurs at any Otterbein location in the MDS Nurse Position. The MDS Nurse Leader will complete the MDS Nurse duties at assigned location. When an Interim role is not needed, then the MDS Nurse Leader will be assigned special projects by the VP Clinical Quality.

Responsibilities

MDS

Qualifications

• Education: BSN
• Experience: Minimum 5 years' of MDS experience in long-term care//nursing home environment required. Experience in post-acute environments also a plus

Special Skills and Requirements

• Must have a proven track record of strong MDS skills (case mix management, PDPM management, Quality Measures)
• Ability to be flexible and adapt quickly to change.
• Experience with Electronic Medical Records; PCC experience highly preferred
• Must be detailed oriented and have excellent literacy and writing, documentation, communications and interpersonal abilities.
  Must be able to cope with mental and emotional stress related to working with persons who are ill, disabled, elderly and/or emotionally upset.
• Must be able to communicate in English.
