Job summary

Pavilion Management Company is seeking a Licensed Practical Nurse (LPN) for a full-time position in Canton, OH. The role involves leading the inter-disciplinary care plan team and coordinating MDS-related activities, with a strong focus on timely care plans and resident well-being.

The position requires an active LPN license, CPR certification, and at least two years of MDS experience in long-term care, plus leadership and organizational abilities. Negotiable salary with benefits offered.

Qualifications

• Current valid nursing license in the state where practice occurs.
• Two years of MDS experience in long-term care.
• Demonstrated leadership and organizational skills.
• Thorough knowledge of nursing principles and procedures.
• CPR certification required.

Responsibilities

• Act as chairperson of the inter-disciplinary care plan team.
• Develop and update plan of care for each resident.
• Establish initial plan of care within five days of admission.
• Maintain and update the plan of care at least biweekly.
• Coordinate care plan meetings with resident/rep participation.
• Meet daily with Therapy Director to review therapy participation and assessment schedule.
• Notify IDT of significant changes requiring full resident assessment.
• Coordinate and lead the weekly Utilization Review/Medicare Meeting.
• Complete monthly skilled billing log and submit by the 8th of the month.
• Ensure emergency and resident care measures are used to stabilize residents.
• Assist in staffing emergencies with clinical tasks as needed.
