Job summary

Regency at Lansing West is seeking an experienced MDS Nurse to manage MDS assessments, care planning, and PPS coordination. You will collaborate with physicians, nursing, social services, therapy, dietary, and activity staff to ensure compliant, patient-centered care.

Ideal candidates hold an RN license and have strong communication and teamwork skills, with experience in RAI and PPS processes. This role is based in Lansing, MI, and emphasizes autonomous work within a collaborative team.

Qualifications

• Experience as an MDS nurse with knowledge of RAI and PPS.
• Registered nurse with ability to work autonomously and manage care plans.
• Strong communication and teamwork abilities within a regulated care environment.

Responsibilities

• Completes the MDS, CAA's and care plans within regulated time frames.
• Assesses resident through physical assessment, interview and chart review.
• Discusses resident care needs with caregivers, including physician, nursing, social services, therapy, dietary, and activity staff.
• Reviews information from hospital, consults and outside agencies for assessment and care planning.
• Coordinates, identifies, and initiates significant changes in MDS.
• Prepare PPS meetings and monitor Medicare days and RUGs utilization.
• Stay current with AANAC requirements.

Responsibilities

• Completes the MDS, CAA's and care plans within regulated time frames.
• Assesses resident through physical assessment, interview and chart review.
• Discusses resident care needs with care givers, including physician, nursing, social services, therapy, dietary, and activity staff.
• Reviews information from hospital, consults and outside agencies and uses such information in the completion of the assessment and care planning.
• Coordinates, identifies, and/or initiates significant change MDS'
• Is prepared to conduct PPS meetings maintaining MDS assessments per Medicare schedule and maintains PPS board for monitoring of Medicare days and RUGs utilization in the absence of the Care Management Coordinator
• Remains current with the American Association of Nursing Assessment Coordinators (AANAC) requirements.

Qualifications

• Registered Nurse, RN
• Knowledge of the Resident Assessment Instrument (RAI) process, including the principles the Prospective Payment Process (PPS) strongly preferred.
• Experience as an MDS Nurse
