Under the direction of the Director of Nursing, the Resident Assessment Coordinator (RAC) leads an interdisciplinary team using a core set of screening, clinical, and functional status elements that serve as the foundation of a comprehensive resident assessment. The RAC utilizes this assessment process to ensure each resident's plan of care reflects their individual needs, goals, and preferences.
The ideal candidate is a nurse with strong assessment and documentation skills who is detail-oriented, organized, and able to think critically. This individual enjoys collaborating with interdisciplinary teams and helping ensure residents receive high-quality, person-centered care.
