Job summary

Daytona Beach Health and Rehabilitation Center is seeking an experienced RN to serve as MDS Coordinator (RN). This daytime, Monday–Friday role emphasizes assessment, care planning, and interdisciplinary collaboration with less direct resident care.

Under the Director of Nursing, the RAC leads resident assessments to ensure care plans meet individual needs and goals, enabling high-quality, person-centered care across teams.

Responsibilities

  • Coordinate interdisciplinary assessment process for assigned residents.

  • Identify and communicate variances to Nursing Director and Care Plan Team.
  • Collaborate with nursing, therapy, dietary, social services, and other departments.
  • Ensure assessment and documentation requirements are completed accurately and timely.

MDS Coordinator (RN)

As an MDS Coordinator (RN), you'll play a key role in resident assessment, care planning, and interdisciplinary collaboration while enjoying a daytime schedule and significantly less direct resident care than traditional bedside nursing positions.

This is an excellent opportunity for a nurse who enjoys assessment, documentation, critical thinking, and care coordination while seeking greater work-life balance.

Under the direction of the Director of Nursing, the Resident Assessment Coordinator (RAC) leads an interdisciplinary team using a core set of screening, clinical, and functional status elements that serve as the foundation of a comprehensive resident assessment. The RAC utilizes this assessment process to ensure each resident's plan of care reflects their individual needs, goals, and preferences.

The ideal candidate is a nurse with strong assessment and documentation skills who is detail-oriented, organized, and able to think critically. This individual enjoys collaborating with interdisciplinary teams and helping ensure residents receive high-quality, person-centered care.

The Details
  • Coordinate the interdisciplinary assessment process for assigned residents.

Qualifications
  • Current Registered Nurse (RN) license in good standing
