Job summary

Diablo Post Acute in California is seeking an experienced RN MDS Coordinator to join our dedicated team. This full-time role focuses on completing accurate MDS assessments and care plans, while ensuring CMS compliance and timely documentation.

You will collaborate with the interdisciplinary team to optimize resident outcomes and participate in quality improvement initiatives in a fast-paced skilled nursing environment. We offer competitive benefits and growth opportunities.

Qualifications

• Valid California RN license required.
• Experience with MDS in a skilled nursing facility preferred.
• Strong knowledge of Medicare, Medicaid and CMS regulations.
• Excellent documentation and communication skills.

Responsibilities

• Coordinate and complete MDS assessments in accordance with state and federal regulations.
• Ensure accurate and timely completion of resident assessments and care plans.

$100,000-$120,000 per year
