Diablo Post Acute in California is seeking an experienced and compassionate Registered Nurse (RN) for the MDS Coordinator role on a full-time basis. You will coordinate MDS assessments, ensure timely care plans, and collaborate with the interdisciplinary team to optimize resident outcomes.

Qualifications
