Job summary

PACS Group is seeking an experienced MDS Nurse (LVN/LPN) to coordinate MDS assessments, develop individualized care plans, and ensure regulatory compliance within a skilled nursing and post-acute setting.

You will work with an interdisciplinary team to support reimbursements, update resident care plans, and educate staff on MDS standards while maintaining accurate documentation and high-quality resident care.

Qualifications

• Current, active LVN/LPN license in the state of practice.
• Knowledge of MDS 3.0, the RAI process, and Medicare/Medicaid reimbursement guidelines.
• Strong organizational skills and exceptional attention to detail.
• Ability to work effectively with interdisciplinary teams.
• Proficiency in electronic health records (EHR) and Microsoft Office Suite.
• Able to interpret clinical information, follow regulatory guidelines, and problem solve.

Responsibilities

• Complete MDS assessments and Care Area Assessments (CAAs) accurately and within regulatory timelines.
• Ensure clinical documentation supports individualized care planning and appropriate reimbursement.
• Maintain CMS and state regulation compliance and facility policies.
• Collaborate with nursing, therapy, social services, dietary, and other team members on care plans.
• Support Medicare/Medicaid reimbursement through accurate MDS submissions.
