Job summary

Western Slope Healthcare Center in Placerville, CA, is seeking an MDS Nurse (LVN/LPN) to join our team. The MDS Nurse coordinates and completes Minimum Data Set (MDS) assessments, ensuring regulatory compliance and appropriate reimbursement.

Qualifications

• Must hold current LVN/LPN license in California.
• Experience with MDS/RAI and Medicare/Medicaid reimbursement preferred.
• Strong organizational skills with attention to detail and accuracy.

Responsibilities

• Complete MDS assessments and CAAs within required timelines.
• Maintain accurate documentation to support care planning and reimbursement.
• Ensure CMS/state regulatory compliance and timely submissions.
• Coordinate the RAI/MDS process from assessment through submission.
• Collaborate with interdisciplinary team to develop individualized care plans.
• Review documentation for consistency and accuracy.
• Provide education on MDS requirements and care planning.
