Qualifications
- In-state Registered Nurse license required (Massachusetts).
- Experience with MDS processes and Medicare/Medicaid guidelines.
- Experience in long-term/sub-acute care and memory care settings.
- Strong communication with residents, families and healthcare teams.
- Knowledge of HIPAA and state/federal regulations.
Responsibilities
- Facilitate MDS documentation and CMS transmissions.
- Coordinate interdisciplinary care planning and updates.
- Conduct resident interviews and assessments within regulatory timeframes.
- Support case management and care transitions.
- Collaborate with PT/OT/SUD teams as needed.
- Maintain accurate records in PCC EMR and ensure HIPAA compliance.
