Job summary

Sapphire Care and Rehabilitation is seeking an MDS Nurse for our Columbus, Ohio center. You will ensure accurate MDS submissions, oversee regulatory compliance, and coordinate care through IDT meetings. Prior MDS experience is required and Ohio RN/LPN licensure is essential.

Proficiency with PointClickCare is a plus, and the role offers full-time hours with flexible scheduling to support positive resident outcomes.

Qualifications

• MDS experience in a long-term care setting is required.
• Knowledge of regulatory requirements and compliance processes.
• Active Ohio RN or LPN license in good standing.
• Experience with PointClickCare is a plus.

Responsibilities

• Ensure accurate and timely MDS submissions.
• Monitor and improve resident outcomes and financial results.
• Oversee regulatory compliance at the center.
• Coordinate IDT meetings and use the PointClickCare system.
