POSITION SUMMARY:

The MDS Coordinator will assist the Director of Nursing in ensuring documentation compliance with federal, state, and certification guidelines. They will coordinate the RAI process, ensuring the timeliness and completeness of MDS, CAAS, and Interdisciplinary Care Plans.

QUALIFICATIONS:

• Must hold a current LVN license
• Minimum of two (2) years nursing experience in a skilled nursing facility preferred
• Excellent knowledge of the RAI Process, Medicare PPS, and Medicaid reimbursement
• Thorough understanding of the Quality Indicator process
• Knowledge of OBRA regulations and the Minimum Data Set
• Knowledge of the care plan process

ESSENTIAL FUNCTIONS:

• Ensure resident safety
• Assist in adherence to regulations and certification standards
• Participate in survey processes and deficiency corrections
• Report audit trends to the Quality Assurance Committee
• Manage the RAI process from MDS to care plan completion
• Monitor RAI tracking, discharge/re-entry, and Medicaid forms
• Ensure documentation compliance and proper signatures
• Coordinate scheduling for care planning conferences
• Support interdisciplinary team meetings and communication
• Maintain confidentiality and timely submission of assessments
• Stay current on industry changes and assist with audits
• Utilize electronic medical records effectively

PHYSICAL REQUIREMENTS:

Ability to perform movements such as pushing, pulling, lifting (up to 50 pounds), bending, kneeling, reaching, with or without accommodations.

WORK SCHEDULE:

As assigned, including some weekends, evenings, and holidays; non-exempt position
