Job summary

American Medical Associates in Warren, Ohio seeks an MDS Nurse as an MDS Coordinator for a long-term care facility. You will complete assessments, MDSs, and care plans for all residents assigned, ensuring timely and accurate documentation.

You will monitor MDS submissions by other disciplines, advise on incomplete assessments, generate monthly care plan calendars, and lead care plan conferences, while demonstrating strong leadership and in-depth MDS 3.0 knowledge.

• Generates and distributes monthly care plan calendar for the following month.
• Conducts care plan conferences for assigned residents.
