Job summary

Apploi is seeking an experienced MDS Nurse (LPN) to coordinate the RAI process and implement each resident’s comprehensive care plan. You will lead MDS assessments, schedule ARDs, and ensure documentation supports accurate MDS coding.

In this role, you will collaborate with health plans and physicians, conduct care plan meetings, and act as the go-to resource for SNF PPS and quality reporting. A current LPN license and RAC training within six months are preferred.

Qualifications

About the role

You’ll conduct and coordinate completion of the Resident Assessment Instrument (RAI), and you’ll be responsible for implementing and evaluating each resident’s comprehensive plan of care.

What you need

• Current, active, unencumbered LPN license
• RAC training within six months of hire (certification preferred)
• Current knowledge of regulations and standards of care
• Intermediate computer skills, including Word, Excel, and EMR systems
• Able to work independently and manage your time effectively
• Effective verbal and written communication skills
• Patience and tact with residents, families, and staff
• Supportive team member and an example of teamwork
• Able to work extra hours, weekends, and holidays when needed

Physical requirements

• Move intermittently throughout the workday
• Bend, stoop, kneel, crouch, and lift overhead as needed
• Assist with resident evacuation during an emergency
• Meet facility health requirements, including exams and immunity status
