Job summary

Apploi seeks an MDS Nurse (LPN) to coordinate RAIs and MDS assessments for residents. You will support development of comprehensive care plans, lead care plan meetings, and ensure documentation meets regulatory standards.

You will work with health plans and physicians, schedule ARDs, and audit charts to maintain accurate MDS coding and quality reporting. Strong communication and independent work ethic are essential.

Qualifications

• Active LPN license in good standing.
• RAC training within six months of hire (certification preferred).
• Knowledge of regulations and standards of care for SNF PPS/RAI processes.
• Proficient with Word, Excel, and EMR systems.

About the role

You’ll conduct and coordinate completion of the Resident Assessment Instrument (RAI), and you’ll be responsible for implementing and evaluating each resident’s comprehensive plan of care.

What you need

• Current, active, unencumbered LPN license
• RAC training within six months of hire (certification preferred)
• Current knowledge of regulations and standards of care
• Intermediate computer skills, including Word, Excel, and EMR systems
• Able to work independently and manage your time effectively
• Effective verbal and written communication skills
• Patience and tact with residents, families, and staff
• Supportive team member and an example of teamwork
• Able to work extra hours, weekends, and holidays when needed

Physical requirements

• Move intermittently throughout the workday
• Bend, stoop, kneel, crouch, and lift overhead as needed
• Assist with resident evacuation during an emergency
• Meet facility health requirements, including exams and immunity status
