The Laurels of Bedford in Michigan seeks an experienced Care Management Nurse, MDS Nurse, to oversee the RAI process and coordinate care plans for residents. You will work with physicians, nursing, social services, therapy and dietary staff to ensure comprehensive assessments and timely MDS completion.

Ideal candidates are organized, autonomous, and capable of leading PPS meetings while staying current with AANAC requirements and Medicare schedules.

Qualifications

• RN or LPN license is required.
• Knowledge of the RAI process, PPS preferred.
• Experience as an MDS Nurse.

Responsabilités

• Complete MDS, CAAs and care plans within regulatory time frames.
• Assess residents via physical assessment, interview and chart review.
• Discuss resident care needs with care givers, physicians, nursing, social services and therapy staff.
• Review information from hospital, consults and outside agencies for assessments and care planning.
• Coordinate, identify, and/or initiate significant changes to MDS.
• Prepare PPS meetings and maintain PPS board for Medicare days and RUGs utilization.
• Stay current with AANAC requirements.

Responsibilities

The Care Management Nurse, MDS Nurse works the RAI process and conducts assessments and care plan coordination for those residents assigned. Some responsibilities of the MDS nurse include:

• Completes the MDS, CAA's and care plans within regulated time frames.
• Assesses resident through physical assessment, interview and chart review.
• Discusses resident care needs with care givers, including physician, nursing, social services, therapy, dietary, and activity staff.
• Reviews information from hospital, consults and outside agencies and uses such information in the completion of the assessment and care planning.
• Coordinates, identifies, and/or initiates significant change MDS'
• Is prepared to conduct PPS meetings maintaining MDS assessments per Medicare schedule and maintains PPS board for monitoring of Medicare days and RUGs utilization in the absence of the Care Management Coordinator
• Remains current with the American Association of Nursing Assessment Coordinators (AANAC) requirements.

Qualifications:

• Registered Nurse (RN) or Licensed Practical Nurse (LPN)
• Knowledge of the Resident Assessment Instrument (RAI) process, including the principles the Prospective Payment Process (PPS) strongly preferred.
• Experience as an MDS Nurse
