Job summary

The City of Lincoln seeks a qualified MDS Nurse Coordinator to oversee the Minimum Data Set process in a Massachusetts facility. The role focuses on overseeing MDS assessments, RAPs, and interdisciplinary care plans within regulatory timelines.

With MA RN licensure and prior MDS experience, you will lead a team, ensure documentation accuracy, and support quality improvement initiatives in a resident-centered care environment.

Qualifications

  • Prior MDS experience required.
  • Certification as Nurse Assessment Coordinator preferred.
  • Knowledge of long-term care laws and regulations.

Responsibilities

  • Develop a schedule for timely completion of the MDS process.
  • Coordinate and lead interdisciplinary care plan meetings.
  • Complete nursing sections of the MDS and RAPs when applicable.
  • Ensure timely completion and signing of MDS sections.
  • Oversee transmission of MDS to state automation databases.
  • Review submission validation reports and act on corrections.
  • Generate monthly Quality Indicator reports for administration.
  • Participate in facility surveys by government agencies.
  • Develop orientation material for MDS process.

Essential Duties and Responsibilities
  • Develop a schedule for timely completion of the MDS process according to federal and state regulations.
  • Schedule, coordinate and lead the interdisciplinary care plan meetings.
  • Complete the nursing sections of the MDS assessment and nursing RAPs when applicable.
  • Assure accurate and timely completion of the MDS assessments according to federal and state regulations.
  • Oversee timely signing of MDS sections completed by all interdisciplinary members.
  • Review all MDS assessments and certify completion by signing appropriate MDS sections.
  • Timely transmission of MDS assessments to the state automation database in accordance with state and federal regulations.
  • Receive and review the submission validation report acing upon any corrections needed or assessments rejected.
  • Review the state monthly reports of assessments submitted and act upon any corrections needed.
  • Generate monthly QI (Quality Indicator) reports and furnish copies to the Administrator and Director of Nurses upon request.
  • Participate in all facility surveys conducted by authorized government agencies.
  • Develop and implement for general orientation a brief overview of the MDS (Minimum Data Set) process.
  • Committed to working with residents and families to develop and implement strategies that promote person-centered care.
  • Applies “5 Star” customer service approach in all aspects of job.
  • Adheres to all company policies, procedures, and standards of conduct.
  • Reports to the appropriate managers any known or suspected violations of policy and procedures, regulations or standards of conduct.
  • Attends required in-services and training.
  • Performs additional duties as assigned.

The MDS Nurse coordinates all components of the MDS process (Minimum Data Sheet) which includes the MDS (Minimum Data Sheet), Triggers, Resident Assessment Protocols (RAPs) and interdisciplinary care plans part of the resident Assessment Instrument (RAI) in compliance with federal and state regulations.
