Job summary

Plaza Health seeks an MDS Director to oversee the Resident Assessment Instrument processes, ensuring timely MDS submissions, CAAs, and care plans in line with CMS, OBRA, Medicare, Medicaid, and NJDOH regulations.

You will collaborate with an interdisciplinary team to maximize appropriate reimbursement, maintain compliance, and promote quality resident care through accurate assessments, audits, and ongoing staff education.

Qualifications

• RN license in NJ preferred; LPN may be considered per state regulations and facility policy.
• 2–3 years of clinical nursing experience, preferably in a skilled nursing facility.
• Previous MDS Coordinator or Resident Assessment Coordinator experience preferred.
• RAC-CT certification preferred but not required.
• Thorough knowledge of the RAI Manual, MDS 3.0, PDPM, Medicare, Medicaid, OBRA, and CMS regulations.
• Experience with electronic medical records, preferably PointClickCare.

Responsibilities

• Coordinate the timely completion of OBRA, Medicare, Medicaid, PPS, IPA, Significant Change, Significant Correction, and Discharge MDS assessments.
• Review resident documentation to ensure MDS coding accurately reflects the resident's clinical condition.
• Maintain the MDS schedule and assessment calendar.
• Monitor ARDs to optimize reimbursement while maintaining regulatory compliance.
• Coordinate interdisciplinary care plan meetings.
• Develop and revise individualized care plans based on assessments.
• Ensure care plans are current and measurable.
