Job summary

River City Rehab in Jacksonville is seeking an experienced Registered Nurse to coordinate and direct resident assessments using the MDS process. You will lead data collection, ensure compliance with Medicare requirements, and collaborate with nursing, therapy, nutrition, and other disciplines to develop care plans.

The role requires a Florida RN license, an MDS certification course (desirable), and CPR within 90 days.

Qualifications

• Education: Associate or bachelor’s degree in nursing, or equivalent is required.
• Completion of accredited MDS certification course is desirable.
• Florida Registered Nurse (RN) license in good standing is required.
• CPR certification within 90 days after hire is required.
• State background check standards must be maintained as a condition of employment in accordance with Florida statutes.

Responsibilities

• Ensure MDS assessments are conducted and implemented for all residents.
• Plan, schedule, and coordinate the work of one or more MDS Coordinator(s).
• Serve as the facility SME for the MDS program.
• Interview residents, residents’ representatives and others with information related to the MDS assessment.
• Examine residents to collect information related to the MDS assessment.
• Collect all data necessary to complete MDS forms and related documentation.
• Create and maintain records of MDS assessments.
• Work with nursing, therapy, nutrition, etc., to ensure care plan guidelines are followed and compliant with Medicare requirements.
• Responsible for setting RUG levels for each resident and ensuring accurate reimbursement.
• Make appropriate referrals to other health care professionals.
• Participate and/or conduct interdisciplinary meetings.
• Assure resident safety through adherence to protocols and open communication.

Position Purpose:

The purpose of this position is to coordinate and direct resident assessments for the facility. Additionally, performs resident assessments of physical and mental function and documents data on minimum data set forms completely and accurately; documents all additional assessments required completely and accurately; and determines appropriate referrals to other health care professionals; and uses the resident assessment protocols to determine whether to proceed or not proceed.

Essential Job Functions – Principal Accountabilities:

• Ensure MDS assessments are conducted and implemented for all residents.


• Plan, schedule, and coordinate the work of one or more MDS Coordinator(s).


• Serve as the facility Subject Matter Expert (SME) for the MDS program.


• Interview residents, residents’ representatives and others with information related to the minimum data set assessment.


• Examine residents to collect information related to the minimum data set assessment.


• Collect all data necessary to complete minimum data set forms and related documentation.


• Create and maintain records of MDS assessments.


• Work with nursing, Therapy, nutrition, etc., to ensure care plan guidelines for each resident are followed, and compliant with ethical standards and Medicare requirements.


• Responsible for setting Resource Utilization Group (RUG) levels for each resident and ensuring the facility is getting accurate, and maximum, reimbursement.


• Make appropriate referrals to other health care professionals.


• Participate and/or conduct interdisciplinary meetings.


• Assure resident safety through strict adherence to protocols, policies, and procedures, open communication with residents, supervisors, and coworkers, and careful observation of residents, equipment, and environmental conditions.

Requirements

Minimum Requirements

Education: Associate or bachelor’s degree in nursing, or equivalent is required. Completion of accredited MDS certification course is desirable.

Experience: Experience performing the essential functions of the position is desirable.

Certification/License: Florida Registered Nurse (RN) license in good standing is required. Candidates are required to attain and maintain CPR certification within 90 days after hire. State background check standards must be maintained as a condition of continued employment in accordance with Florida Statutes Chapters 435 and 408.809 and Florida Administrative Code 59A-35.090.
