Lake Country Health Services is seeking an experienced MDS Director (RN) to lead the Resident Assessment Instrument (RAI) process and ensure accurate assessments for Medicare/Medicaid and other payers in our skilled nursing center in Oconomowoc, WI.

The ideal candidate will have RN licensure in good standing and 2+ years of MDS experience in post-acute care, with strong PDPM knowledge and leadership abilities to support an engaged interdisciplinary team.

Lake Country Health Services is seeking an experienced MDS Director (RN) to lead the assessment and reimbursement program in our respected skilled nursing center located in beautiful Lake Country / Oconomowoc.

Role Overview

The MDS Director leads the Resident Assessment Instrument (RAI) process and ensures accurate and timely completion of resident assessments in accordance with Medicare, Medicaid, OBRA, and other payer requirements. This role works closely with nursing leadership, therapy, and administration to ensure assessments accurately reflect each resident’s clinical condition while supporting regulatory compliance and reimbursement accuracy.
