Qualifications

• Excellent communication with patients, families, and healthcare professionals.
• Proven leadership and management skills, with ability to motivate teams.

Responsibilities

• Oversee the entire MDS process, ensuring compliance with federal and state regulations and accrediting agency standards.
• Develop and implement policies and protocols for timely completion of MDS assessments, care plans, and care delivery.
• Conduct thorough MDS assessments of patients' physical, psychological, and social needs to identify care improvements.
• Collaborate with the interdisciplinary team to develop individualized care plans addressing patient needs and goals.
• Directly supervise and mentor MDS coordinators, therapists, and other healthcare professionals to ensure high-quality care.
