Job summary

Arcadia Care is seeking an MDS Coordinator to oversee Medicare/Medicaid case-mix documentation, MDS assessments, and care plan accuracy to ensure proper reimbursement in a skilled nursing setting. The role requires RN or LPN licensure, long-term care experience, restorative certification within six months, and strong English communication.

You will coordinate interdisciplinary teams and ensure timely submissions.

Qualifications

• RN or LPN licensure required per state credentialing.
• Long-term care experience is required.
• Restorative Certification must be obtained within six months of hire.
• English language proficiency (reading/writing/speaking) required.

Responsibilities

• MDS Assessment and Documentation: assess health status and care level for new admissions and ensure timely MDS submissions.
• Care Planning and Resident Assessment: prepare care plans on admission, quarterly, and as needed per regulatory timeframes.
• Care Conferences and Interdisciplinary Coordination: schedule and conduct MDS reviews prior to conferences.
• Restorative Nursing Program Coordination: oversee restorative nursing measures on the care plan.

MDS Coordinator

Job Summary:

The MDS Coordinator is responsible for the accurate and timely completion of all Medicare/Medicaid case-mix and care plan documentation in order to assure appropriate reimbursement for care and services provided within the Facility. Conducts continual Minimum Data Set (MDS) and care plan reviews to assure completeness, thoroughness, and achievement of the optimal allowable Patient Driven Payment Model (PDPM) and Resource Utilization Group (RUG) categorization. Oversees the overall process and tracking of MDS/Prospective Payment System (PPS) documentation and submission, care plan documentation, and the restorative nursing program. He/she will integrate nursing, dietary, social recreation, restorative, rehabilitation and physician services to ensure appropriate assessment and reimbursement.

Essential Duties:

• MDS Assessment and Documentation
  • Assesses and determines the health status and level of care of all new admissions.
  • Ensures the accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, significant change.
  • Maintains an accurate schedule of all MDS assessments to include the proper reference dates throughout the resident’s stay.
• Care Planning and Resident Assessment
  • Completes care plans on admission, quarterly and as needed for each resident according to regulatory time frames. Ensures completeness and thoroughness of documentation as mandated by federal and state standards.
• Care Conferences and Interdisciplinary Coordination
  • Schedules and conducts resident care conferences in compliance with state and federal regulations and ensures completion of all MDS reviews prior to resident care conference.
• Restorative Nursing Program Coordination
  • Oversees that all restorative nursing measures are reflected on the care plan as an approach to the focus or need for which they are being completed.

Preferred Qualifications:

• Registered Nurse or Licensed Practical Nurse with current unencumbered state licensure.
• Long Term Care Experience required.
• Restorative Certification required within six (6) months of hire if not already certified at time of employment.
• Ability to read, write, speak and understand the English language.
