Qualifications

  • Must have RN or LPN licensure.

We are seeking an experienced MDS Nurse (RN or LPN) with a sign-on bonus of $10,000 for FT RNs and $7,500 for FT LPNs. The MDS Coordinator oversees the RAI process, conducts assessments, and coordinates care plans, supervising the Care Management Nurse and MDS Nurse.

Responsibilities
  • Complete MDS, CAA’s, and care plans within regulated time frames.
  • Coordinate scheduling of the RAI process with the interdisciplinary team.
  • Assess residents through physical exams, interviews, and chart reviews.
  • Discuss resident care needs with caregivers, including physicians and staff.
  • Review information from hospitals and outside agencies for assessments and care planning.
  • Initiate significant change MDSs and conduct PPS meetings, maintaining compliance with Medicare schedules.
  • Stay current with AANAC requirements.
Qualifications

  • Knowledge of RAI process and PPS principles
  • Experience as an MDS Nurse
