Job Details Job Location: Pilgrim Place In Claremont - Claremont, CA Position Type: Full Time Salary Range: $45.00 - $55.00 Hourly

Pilgrim Place in Claremont, a vibrant senior community committed to Justice, Peace and Care of the Earth, seeks an experienced MDS Coordinator (Minimum Data Set). This position will work closely with and report to the Director of Nursing. With our history of over 110 years of serving seniors from not only our local, but greater world-wide community, we are excited to welcome you to our family!

Key Responsibilities include:

• Demonstrate to all staff, residents, and families that the organization is committed to the creation of the pleasures and good daily life expected of home.
• Model and ensure that others are providing medical treatment in a manner that honors resident preferences and individuality. Demonstrate strong commitment to the highest clinical outcomes.
• Recognizes early signs of changing conditions and uses all available and appropriate resources to resolve needs. Very strong observational skills with regards to physical and medical conditions, coupled with ability to discern effective response and treatment.
• Ability to model person-centered practices, as well as evaluate and provide direction and coaching to other staff as needed.

Ability to provide clear directions and explanations and elicit trust and cooperation from staff. Ability to communicate residents’ medical needs clearly with staff and families, and to facilitate other staff participation in medical care through eliciting others’ observations and suggestions. Able to communicate clearly and effectively with physicians.

Qualifications

Requirements for the position include:

• Valid LVN or RN license to practice nursing in the state of California.
• Knowledge of Title XXII and OBRA regulations.
• Three to five years experience in nursing (rehabilitative or geriatric).
• Ability to problem solve and provide leadership to the nursing staff.
• Ability to maintain confidentiality in a variety of circumstances and make sound judgments.
