Job Location: New Mexico (Pick your location). We have several new facilities in New Mexico that require an experienced MDS Coordinator.

Your Job Summary

The MDS Coordinator will be responsible for timely and accurate completion of the RAI process and care management process from admission to discharge, in accordance with company policies, procedures, and federal, state, and certification guidelines. This includes Minimum Data Set, discharge and admission tracking, etc. Under the guidance of the Director of Nursing and VP of Clinical Reimbursement, may coordinate information systems operations and education for the clinical department.

Principal Responsibilities
  • Collaborate with the Interdisciplinary Team to assess resident needs; provide schedules for MDS assessments and care plan reviews as required by governing agencies.
  • Ensure the interdisciplinary team makes decisions regarding additional MDS assessments based on clinical criteria per the latest RAI User’s Manual.
  • Assist with coordination of daily stand-up meetings, review resident care, and set assessment reference dates.
  • Comply with federal and state regulations for the RAI process.
  • Monitor MDS and care plan documentation for all residents, ensuring proper documentation supports MDS coding.
  • Maintain current MDS status for residents according to guidelines.
  • Accurately enter resident data into computerized MDS programs.
  • Complete precise coding of MDS based on medical records, observations, and interviews with staff, residents, and families.
  • Participate in interdisciplinary meetings, quality assurance, and other relevant meetings to gather information and update records.
  • Assist with identification of significant changes, physician orders, and verbal reports to ensure MDS and care plans reflect current status.
  • Prepare scheduling and notices for care planning conferences and communicate outcomes to staff, residents, and families.
  • Continuously update knowledge related to data entry and technology.
  • Complete electronic submission of documentation to state databases and other entities as per policy.
  • Ensure the final MDS is completed and resident assessment data is submitted to appropriate agencies.
  • Instruct staff in the RAI process, PPS Medicare, Medicaid, and clinical computer systems.
  • Maintain confidentiality of all information.
  • Perform other duties as assigned.
Qualifications
  • Graduate of an approved RN program and licensed in the state of practice.
  • At least 2 years of nursing experience in a Skilled Nursing Facility preferred.
  • Strong knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement.
  • Understanding of Quality Indicator processes, OBRA regulations, and MDS.
  • Experience with MDS 3.0 is preferred.
