Job description

We are looking to promote and train from within for a compassionate RN or LPN to join our MDS Department. The right candidate has a can-do attitude, a friendly demeanor, excellent interpersonal skills, and the ability to provide medical support to our clients in the facility.

Responsibilities

  • Communication skills to work with nursing staff for implementation of care plan
  • Oversees care plan meeting with resident and family
  • Assures that care plans are keep current and reflective
  • Verifies that assessments transmitted to database with necessary corrections
  • Reviews of the MDS for accuracy for any trends that need education
  • Monitors documentation of Medicare and managed care resident and educate as needed
  • Monitoring of QM with emphasis on those impacting five-star rating
  • Experience in CMI for best outcomes and guidelines for documentation required
  • Knowledge of restorative programs and implementation
  • Participation in Quality Improvement program in the facility
  • Point Click Care experience preferred

Qualifications
