What You'll Do:

As our MDS Coordinator, you’ll be the go-to expert for all things Resident Assessment Instrument (RAI)/Minimum Data Set (MDS). You’ll lead efforts to ensure assessments are completed accurately and on time while collaborating across departments to support comprehensive, resident-centered care plans.

Your Responsibilities:

• Coordinate and oversee completion of resident Interdisciplinary Care Plans
• Partner with nursing staff and other departments for accurate and personalized care planning
• Ensure timely and compliant MDS submissions per CMS guidelines
• Analyze Quality Measures and Indicators using RAI/MDS data
• Champion best practices in assessment and documentation
