$36.00/hr - $41.00/hr

Major Responsibilities
  • Coordinate and participate in completing the Resident Assessment Instrument (MDS, CAA's, and Care Plan) per Federal and State regulations.
  • Monitor and document management of Medicare and Managed Care residents collaboratively with the facility team.
  • Implement innovative healthcare practices to improve systems and achieve superior results.
Qualifications
  • Current, active, unencumbered state license as RN or LPN.
  • Preferred experience in Skilled Nursing/Rehabilitation facilities.
  • Minimum of 6 months experience as an MDS Coordinator.
  • RAC-CT or RNAC certification preferred.
  • Must be qualified, compassionate, and dedicated to quality work.
