MDS Coordinator (RN or LPN) – Cincinnati, OH

A respected Skilled Nursing Facility in Cincinnati, Ohio is seeking a qualified and detail-oriented MDS Coordinator (RN or LPN) to join our clinical team. This is a full-time, on-site opportunity for a nurse who is passionate about clinical documentation and resident-centered care.

Position Summary

The MDS Coordinator is responsible for overseeing the completion of the Minimum Data Set (MDS) assessments and ensuring accurate, timely documentation that supports resident care planning and regulatory compliance. This role works closely with the interdisciplinary team to drive quality outcomes.

Key Responsibilities

  • Coordinate and complete MDS assessments in accordance with federal and state guidelines
  • Ensure accuracy of documentation for reimbursement and compliance purposes
  • Monitor resident conditions and update care plans and assessments accordingly
  • Collaborate with clinical and administrative teams to support quality improvement

Qualifications

  • Active RN or LPN license in the state of Ohio
  • Previous MDS experience in a skilled nursing or long-term care setting required
  • Working knowledge of MDS 3.0, RAI process, PDPM, and Medicare/Medicaid regulations
  • Excellent assessment, documentation, and communication skills
