Description

Primary Responsibilities

Responsible for coordinating the Resident Assessment Instrument (RAI) process to ensure accurate and timely completion of resident assessments in line with Medicare, Medicaid, OBRA, and other payer requirements. Ensures assessments reflect the physical, mental, and psychosocial status of residents, with proper documentation to support services and assessment accuracy. Communicates effectively with the interdisciplinary team.

Follows all RIHS policies and procedures.

Essential Functions
  • Ensures timely, accurate, and complete assessment of residents' health and functional status.
  • Participates in pre-admission processes to gather essential referral information.
  • Completes Medicare/Medicaid case-mix documents accurately and on time for proper reimbursement.
  • Collaborates with the Director of Rehab to determine appropriate assessment reference dates.
  • Tracks Medicare/Managed Care eligibility and benefits using Case Management Tools, coordinating with relevant staff.
  • Gathers information for Managed Care Utilization Reviews and communicates with external Case Managers.
  • Ensures Medicare requirements like Physician certification are met.
  • Performs concurrent MDS reviews to ensure correct RUGs categories.
  • Participates in interdisciplinary team meetings for care planning.
  • Completes all MDS assessments timely and accurately, including PPS, Unscheduled, Admission, Quarterly, Annual, and Significant Change assessments.
  • Identifies significant status changes and manages related assessments.
  • Maintains schedule and proper reference dates for assessments.
  • Tracks default days, implements corrective actions, and audits MDS processes regularly.
  • Ensures electronic submission of MDS and reviews validation reports for accuracy.
  • Addresses late/missed assessments and reviews quality reports monthly.
  • Communicates with the Business Office about case mix and reimbursement impacts.
  • Participates in daily, weekly, and monthly meetings related to case management and care levels.
  • Serves as an RAI and Care Management resource for staff and assists in training.

Qualifications

Educational/Training

  • Graduate of an accredited nursing program with current R.N. or L.V.N. license.
  • LVN candidates may be considered, but specific functions require RN coordination as per regulations.
  • Complete RIHS-specific MDS/RUGs training within 90 days of employment.
  • Proficiency in office and MDS/RAI software applications.

Licensing

  • Valid licensure in the state of employment.

Experience

  • At least two years of healthcare experience.
  • Experience with MDS, reimbursement, and case management preferred.
