Key Responsibilities

• Coordinate the delivery of services to residents in collaboration with facility team members.
• Monitor and document the cost-effectiveness of treatments provided to residents.
• Facilitate and coordinate the admission and discharge process for residents.
• Serve as an advocate for residents and their families, addressing their needs and concerns.
• Act as a liaison between residents, families, and insurance/medical management professionals.
  
  

Requirements

• Nursing Degree from an accredited college or university, or graduation from an approved RN or LPN/LVN program.
• Minimum of three (3) years of experience in a hospital, nursing care facility, or related healthcare environment.
• Current, unencumbered RN or LPN/LVN license to practice in the state.
• Knowledge of managed care rules, regulations, and guidelines, along with rehabilitative and restorative nursing practices.
• Strong communication skills in English, with the ability to make independent decisions and relate information effectively.
