Join Our Team as an MDS Coordinator – Hamilton Grove

Are you a compassionate and detail-oriented nurse looking to make a meaningful impact? Hamilton Grove, part of the Greencroft Communities family, is seeking a dedicated MDS RN \ LPN to join our team. In this role, you’ll play a key part in improving the quality of life for our residents by ensuring timely and accurate completion of MDS assessments and care plans.

Schedule:

• Hourly rate based on experience
• $1.50 additional per hour for second and third shift.
• Weekend shifts pay: $1.50 additional per hour

What You’ll Do:

Complete and coordinate MDS assessments and care plans in accordance with federal guidelines

Monitor and respond to resident changes and clinical issues

Collaborate with the nursing team to ensure high-quality, personalized care

Maintain current documentation and records for compliance and resident well-being

Support restorative nursing programs and ensure timely updates to care plans

What We’re Looking For:

Licensed RN OR LPN in the state of Indiana

Experience in long-term care or retirement communities preferred

Strong assessment, communication, and organizational skills

Familiarity with MDS processes and Medicare requirements
