Position Overview:

The MDS Coordinator supervises and coordinates all MDS (Minimum Data Sets) and care plans for residents, ensuring compliance with state requirements and timely completion.

Key Responsibilities:
  • Conduct and coordinate MDS and Care Plans as outlined by facility policies.
  • Inform care team members of upcoming care plan due dates and ensure completion.
  • Develop and revise resident care plans quarterly or with significant changes.
  • Ensure all PPS and MDS assessments are completed accurately and timely.
  • Coordinate care plan meetings and encourage team participation.
  • Maintain accurate and timely submission of MDS to authorities per guidelines.
  • Observe resident rights, including confidentiality and privacy.
  • Communicate effectively among departments for updates and changes.
  • Monitor resident care during MDS processes and report concerns.
  • Assist with pre-admission screening to estimate RUGs groups.
Qualifications:
  • Active RN or LPN license in the state of employment.
  • Experience in Long Term Care preferred.
  • Proficiency in Data Entry, Word, and knowledge of MDS 3.0.
  • Understanding of nursing practices, laws, and regulations.
  • Ability to plan, organize, and implement care programs.
  • Active CPR license or ability to obtain within 30 days of employment.
