Join Our Team as an MDS RN – Hamilton Grove

Are you a compassionate and detail-oriented nurse looking to make a meaningful impact? Hamilton Grove, part of the Greencroft Communities family, is seeking a dedicated MDS RN to join our team. In this role, you’ll play a key part in improving the quality of life for our residents by ensuring timely and accurate completion of MDS assessments and care plans.

What You’ll Do:
  1. Complete and coordinate MDS assessments and care plans in accordance with federal guidelines
  2. Monitor and respond to resident changes and clinical issues
  3. Collaborate with the nursing team to ensure high-quality, personalized care
  4. Maintain current documentation and records for compliance and resident well-being
  5. Support restorative nursing programs and ensure timely updates to care plans
What We’re Looking For:
  1. Licensed RN in the state of Indiana
  2. Experience in long-term care or retirement communities preferred
  3. Strong assessment, communication, and organizational skills
  4. Familiarity with MDS processes and Medicare requirements
