Fairview Skilled Nursing Home is seeking an experienced MDS Coordinator RN to oversee the MDS/RAI process, ensure accurate assessments, and support regulatory compliance for optimal resident care and reimbursement.

The role requires NY RN licensure and knowledge of MDS 3.0, PDPM, CMS regulations, and strong organizational and communication skills to collaborate with an interdisciplinary team and educate staff on MDS documentation.

Position Summary
