Job Responsibilities
  • Provide compassionate care and support to residents
  • Assist in onboarding new team members
  • Coordinate assessments using RAI tools to support individualized resident care
  • Manage resident care plans and advocate for residents
  • Oversee resident assessments for reimbursement and report transmission
  • Participate in Quality Improvement initiatives to improve resident outcomes
Minimum Qualifications
  • Demonstrate C.A.R.E. values
  • Graduate of an accredited School of Nursing (preferably BSN)
  • At least one year of nursing management experience in long-term care
  • Two years of professional nursing experience in relevant settings
  • Knowledge of Resident Assessment Instrument, Care planning, and MDS 2.0
  • Familiarity with OBRA regulations and current reimbursement guidelines
  • Knowledge of laws affecting nursing facility care
