Qualifications

• Must hold a current RN state license and have long term care experience.
• MDS Coordinator experience is preferred but not mandatory.

Responsibilities

• Coordinates completion of MDS documentation and supervises care plans.
• Schedules RAI completion according to state and federal guidelines.

• Coordinates completion of MDS documentation
• Supervises utilization of CAAs and care plans
• Schedules completion of RAI following state and federal guidelines
• Manages Case Mix Results

QUALIFICATIONS:

• Current RN state license
• 1+ years of long term care experience
• MDS Coordinator experience, preferred
• Excellent communication skills; written and verbal
• Positive Can-Do Attitude
• Excellent Attendance
