Life Care Center of Cheyenne is seeking an RN MDS Coordinator to manage interdisciplinary MDS Assessments, CCAs, and Care Plans in accordance with CMS RAI guidelines. The role requires an active RN license, at least two years of nursing experience, and strong regulatory knowledge.

The coordinator will educate staff, participate in required compliance meetings, and ensure accurate documentation and timely submissions while upholding Life Care standards.

Mansioni

• Coordinate and submit interdisciplinary MDS Assessments, CCAs, and Care Plans per CMS RAI Manual Regulations
• Notify DON of changes in a patient’s condition identified by MDS assessments
• Educate direct care staff on CMS RAI Manual updates and Skilled Nursing Facility regulations
• Review Interdisciplinary Comprehensive Care Plans and Final Validation Reports for CMS QIES compliance
• Attend Daily PPS Meetings, Monthly Triple Check, and other required meetings
• Perform staff nurse duties as needed and maintain professional conduct and confidentiality

Position Summary

The RN MDS Coordinator coordinates and assists with completion and submission of accurate and timely interdisciplinary MDS Assessments, CAAs, and Care Plans according to CMS RAI Manual Regulations and in accordance with all applicable laws, regulations, and Life Care standards.

Education, Experience, and Licensure Requirements

Associate's or bachelor's degree in nursing from an accredited college or universityCurrently licensed/registered in applicable State. Must maintain an active Registered Nurse (RN) license in good standing throughout employment. Two (2) years' nursing experience. Geriatric nursing experience preferred. CPR certification upon hire or obtain during orientation. CPR certification must remain current during employment.

Coordinate and assist with completion and submission of interdisciplinary, accurate, and timely MDS Assessments, CCAs, and Care Plans according to CMS RAI Manual RegulationsReport any changes in a patient's condition identified by the MDS Assessment to the DONProvide education to direct care associates regarding updates or changes to the CMS RAI Manual or Skilled Nursing Facility Regulations that impact documentationAssist with review of the Interdisciplinary Comprehensive Care PlanReview Final Validation Reports and attest that all assessments have been completed and accepted into the CMS QIES system prior to billing and notify the Business Office when assessments are not ready to billReview CMS Reports to identify assessments completed or submitted late and develop systems and processes to prevent reoccurrenceAttend and participate in the Daily PPS Meeting, Monthly Triple Check, and other meetings upon requestPerform functions of a staff nurse as requiredExhibit excellent customer service and a positive attitude towards patientsAssist in the evacuation of patientsDemonstrate dependable, regular attendanceConcentrate and use reasoning skills and good judgmentCommunicate and function productively on an interdisciplinary teamSit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hoursRead, write, speak, and understand the English language
