Allbridge Rehabilitation and Nursing Center is seeking an experienced part-time RN MDS Coordinator to coordinate the RAI process, ensuring timely and complete MDS, CAA’s, and interdisciplinary care plans.
Responsibilities include monitoring assessments, inputting MDS data, coordinating care plan conferences, and facilitating monthly QM meetings. Requires a nursing degree, active RN/LPN license and CPR, plus at least 2 years in long-term care as an MDS Coordinator.
Allbridge Rehab and Nursing is seeking an experienced part-time RN MDS Coordinator. The primary purpose of this position is to coordinate the RAI process assuring the timeliness, and completeness of the MDS, CAA’s, and Interdisciplinary Care Plan.

Monitors and guides the completion of assessments in a timely manner
Involvement and supervision of inputting of MDS assessments, the transmission to the state, and obtains receipt of the validation
Schedules and facilitates care plan conferences Facilitates monthly QM meetings
Manages Care Planning Process
Must hold a Nursing Degree from an accredited college or university
A current, unencumbered, active RN/LPN license and CPR
At least 2 years’ experience in Long Term Care as an MDS Coordinator.
Outstanding knowledge of Case-Mix, Federal Medicare PPS process, and Medicaid reimbursement.
Excellent written and verbal communication skills and critical thinking abilities,
Solid understanding of current federal, state, and local standards, regulations, and guidelines that govern our facility
Ability to organize and prioritize.
