Job summary

Goldwater Care Danville is seeking an MDS Coordinator to ensure accurate and timely completion of Medicare/Medicaid case-mix documentation. You will continually review MDS to optimize RUG categories and assure appropriate reimbursement for services provided.

The role requires RN or LPN licensure with LTC experience preferred and strong English communication. You will coordinate interdisciplinary efforts and maintain regulatory-compliant documentation throughout the resident stay.

Qualifications

• Experience coordinating MDS processes and documentation.
• Ability to collaborate with interdisciplinary care teams.
• Familiarity with Medicare/Medicaid case-mix requirements.

Responsibilities

• Assess health status and care level for new admissions.
• Ensure timely completion of MDS assessments (PPS, quarterly, annual, significant change).
• Communicate level of care to all disciplines.
• Coordinate interdisciplinary participation for MDS within regulatory time frames.
• Maintain schedule of MDS assessments with correct reference dates.
• Enter data accurately and submit MDS electronically; verify submissions.
• Coordinate MDS completion to meet federal/state standards.
