Job summary

Talentify is seeking an experienced MDS Coordinator for a long-term care facility in Albion, NY. You will lead the MDS assessment process with the interdisciplinary team to ensure high-quality resident care and regulatory compliance.

The role requires a New York RN or LPN license, strong organizational and leadership skills, and excellent written and verbal communication. Deep knowledge of MDS 3.0, RAI processes, and care planning is essential.

Qualifications

• Must be licensed as RN or LPN in New York.
• Experience as an MDS Coordinator in a nursing home/long-term care setting.
• Excellent organizational and leadership abilities.
• Strong written and verbal communication skills.
• Solid knowledge of MDS 3.0, RAI processes and care planning requirements.

Responsibilities

• Ensure MDS assessments are accurate and regulatory compliant.
• Coordinate MDS development with the interdisciplinary team.
• Implement compliance and quality programs and procedures.
• Ensure timely completion of MDS, care plans and documentation per regulations.
• Serve as liaison among nursing, rehabilitation, social services, dietary and other teams.
