Job summary

Maplewood Health Care Center in Jackson, TN is seeking a dedicated MDS Coordinator (LPN or RN) to join our care team. This full-time role focuses on accurate RAI/MDS governance, care plan evaluation, and documentation within regulatory guidelines.

The ideal candidate holds Tennessee LPN/RN licensure, has at least 3 years of nursing experience with supervisory responsibilities, and will complete MDS training within 6 months of hire; RAC is preferred.

Qualifications

• Active LPN or RN license in Tennessee
• Minimum 3 years nursing experience with supervisory duties
• MDS training to be completed within 6 months of hire; RAC preferred

Responsibilities

• Attend weekly education meetings to stay updated on MDS changes.
• Coordinate the facility's RAI process in accordance with state and federal guidelines.
• Accurately complete all MDS assessments and supporting documentation.
• Evaluate resident care plans and audit medical records with the interdisciplinary team.
• Perform additional tasks as assigned by Regional MDS Consultants, Administrator, and Director of Nursing.
• Maintain confidentiality of protected health information across verbal, written, and electronic communications.

Skills

MDS Coordinator Position Summary

• Attend weekly educations meetings to stay updated on MDS changes.
• Coordinate the facility's Resident Assessment Instrument (RAI) process in accordance with state and federal guidelines.
• Accurately complete all MDS assessments and any supporting assessments or clinical documentation.
• Evaluation of resident's comprehensive plan of care, auditing medical records for supporting documentation, collaborating with the interdisciplinary team.
• Perform any other additional tasks as assigned by the Regional MDS Consultants, Administrator, and Director of Nursing.
• Maintain confidentiality of protected health information, including verbal, written and electronic communications.

MDS Coordinator Requirements

• Active LPN or RN license for the state of Tennessee
• 3 years nursing experience including supervisory experience
• MDS training must be completed within 6 months of hire
• RAC Certification preferred
