Signature HealthCARE of Norfolk seeks an MDS Candidate who is detail-oriented, organized, and patient-focused to perform resident assessments and coordinate care with families.

You will collect information, participate in PPS/Medicare meetings, review plans of care quarterly, and ensure accurate documentation to meet federal billing requirements while upholding our resident-centered culture.

Competenze

• Active LPN license with current state licensure.
• Minimum three years of clinical experience in a health care setting, long-term care preferred.
• Minimum one year of MDS experience.

Mansioni

• Collect information to complete the MDS using medical records, bedside assessments, and interviews.
• Participate in Daily PPS, weekly Medicare, and month-end meetings to ensure billing compliance.
• Review Plan of Care quarterly and with each Comprehensive Assessment to reflect changes.
