Job summary

Liberty Health in Burlington, North Carolina, is seeking an experienced MDS Coordinator (LPN license required) to manage the MDS process across disciplines, ensuring timely submissions and accuracy of assessments, CAAs, and care plans.

You will coordinate the Resident Care Plan, schedule and conduct RCP meetings, and work with all departments to ensure compliance with state and federal guidelines; background checks and EOE apply.

Qualifications

• Licensed Practical Nurse with an active NC LPN license.
• Experience with MDS-RAP, RUG-IV, and care planning.
• Knowledge of MDS 3.0 and CAAs per state guidelines.

Responsibilities

• Maintains and follows a schedule of due dates for all MDS.
• Coordinates completion of the MDS by all disciplines.
• Reviews the MDS data for accuracy and meets with staff as needed.
• Completes MDS and inputs into the computer.
• Verifies that assessments transmitted and approved by State in a timely manner.
