Qualifications

• Must have an active LPN/VN license.
• 3+ years of clinical experience, preferably in long-term care.
• At least 1 year of MDS experience required.

Responsibilities

• Complete the MDS using medical records and assessments.
• Participate in meetings to meet billing requirements.
• Review and update the Plan of Care quarterly.

Overview

The ideal MDS Candidate is detail-oriented, organized, self-motivated, and engaged in bedside resident assessments and family meetings.

How You Will Make a Difference

• Complete the MDS using medical records, bedside assessments, and interviews.
• Participate in PPS, Medicare, and month-end meetings to meet billing requirements.
• Review and update the Plan of Care quarterly and with each assessment.

What You Need to Make a Difference

• Current LPN/VN license in the state.
• At least 3 years of clinical experience, preferably in long-term care.
• Minimum 1 year of MDS experience.
